You will be scheduled with a patient care coordinator to set up your account and determine your eligibility for financial assistance. Please bring proof of income (income tax return, pay stub, or bank statement).
If you have no income, please call or go to Social & Rehabilitation Services (SRS) to apply for assistance. The SRS will give you a letter stating your status, which you will need to bring with you to your first appointment at PrairieStar.
If you are on Disability, Social Security or Unemployment, please bring a status letter or proof of the assistance you are receiving.
