PrairieStar Health Center offers a variety of services to meet your industrial healthcare needs. Whether it is a DOT Physical, Drug Urinalysis, Breath Alcohol Testing, Pulmonary Function Testing or Workers’ Compensation, we have what you need to get the job done. We will provide you with exceptional services at an affordable price. In most cases, we will accommodate same day care. Monday through Thursday: 8:00am to 5:00pm and Friday: 8:00am to 3:00pm
In addition to being a full service Family Practice Clinic, we provide both Federal and Non Federal Drug Testing. This includes pre-employment, random, return to duty, follow-up, post accident, reasonable suspicion/cause, and promotion in a variety of panel sizes to meet your individual business needs. We can also add urine alcohol if this is desired.
We provide full DOT Approved Breath Alcohol Testing. We utilize the Alco-Sensor IV with Memory from Intoximeters, Inc. They have been the world’s leader in breath alcohol testing for over sixty years.

Collection Fee Only (Drug Urine) $15.00 (if laboratory affiliated)
Breath Alcohol Testing (BAT) $25.00 (includes confirmation if needed)
NON DOT In-House Drug Screening $25.00 Urine (10 panel)/Oral Fluid (6 panel) (immediate findings to employer)
Observed Urine Collection (DOT/NON DOT) No Additional Charge
Hair Sample Collection $25.00 (Employer/Donor Must Provide Kit)
Walk-ins are welcomed and will be handled in an expeditious manner. We are staffed with 6 certified collectors and 4 Breath Alcohol Technicians to reduce your waiting time. PrairieStar provides DOT Physicals for only $45.00 which includes the routine urinalysis. We provide full laboratory services as well. We have united with Affiliated Medical Services (AMS) Clinical Reference Laboratory for laboratory studies not performed by our in house laboratory staff.
PrairieStar Health Center utilizes the IQmark Digital Spirometer and its advanced technology to meet your needs in pulmonary assessment and screening.
We currently contract with LabCorp, Clinical Reference Laboratory (CRL), Advanced Toxicology Network (ATN), Quest Diagnostics, MedTox, and South Bend Medical Foundation. We can also work with any Third Party Administrator (TPA) or Consortium that you may be associated with.
We will provide collection only services to any client if they have the appropriate chain of custody form/kit for $15.00.
